N.M i think if u r unhealthy them u can pay or make some poor person to get food for seher and iftar , but on only one condition that if u r really not fit and healthy for fasting ..... i know this but , ask ur friend to ask with some elder about it

N.M yes dear , islam permits unhealthy person to skip fasting, but as i said she/he should pay to a poor or provide seher and iftar's food to a poor who is fasting , ask ur friend to do this and she can skip fasting
